  • Peer Recovery Support Specialist, Family Medicine

    The Primary Care Research Institute (PCRI) in the Department of Family Medicine seeks a Peer Recovery Support Specialist to support others in recovery from a substance use disorder. Funding will be provided by a grant awarded by the Erie County Department of Health on behalf of the Erie County Opioid Epidemic Task Force: 'Peer support workers to improve access to substance use treatment and recovery services in primary care settings.

    This individual will serve as an advocate and motivator to recovering individuals to help promote long-term recovery. The Peer Support Specialist will develop trusting relations with patients while maintaining professional boundaries, help clients address challenges during the recovery process, and assist clients in linkage to and engagement in care. In addition, the peer will integrate with an interdisciplinary primary care team, to address barriers to care in a trauma -informed manner.

    Duties and responsibilities include:



    • Conduct outreach to establish and cultivate relationships with patients and organizations.

    • Professionally represent the clinic and providers to clients, community members and community organizations.

    • Conduct intake and ongoing assessment.

    • Assess individual needs, identify barriers to successful connection to and retention in care, and coordinate with care team to connect clients to services as appropriate.

    • Handle the logistics for patients such as arranging transportation for the client, managing follow-up appointments (such as help with scheduling and reminders), and collaborating with other care team members.

    • Work with provider office to develop policies and procedures related to integrating a peer model in a primary care setting.
