Departmental Overview

UC Berkeley's University Health Services (UHS) is a comprehensive college health service providing fully accredited, primary medical care, counseling and psychological services, and innovative health promotion programs for students, faculty, and staff. UHS provides on-campus medical care and coordinates supplemental needs for off-campus care through a network of community specialists and hospitals. UHS manages the student health insurance and workers' compensation insurance programs.

Approximately 65,000 visits occur annually including Primary Care, Urgent Care, Occupational Health and Specialty Clinics. Services are designed to minimize the impact of illness, emotional distress and injury on studies and work. The Social Services (SOS) unit offers short-term counseling, groups, case management and nutrition services to the campus student population. Counseling staff consists of social workers who provide compassionate specialty counseling on a range of specific and highly sensitive topics. Responsibilities

CLINICAL DUTIES

A. Patient Care
  • Utilizes the nursing process as a frame of reference for practice: has the authority, responsibility and accountability for making consistently sound clinical decisions for complex patient care management specific to nursing assessment, planning, implementation and evaluation through appropriate communication modalities. Recognizes, interprets and reports abnormal patient findings and determines when patient is at risk or potentially at risk and consults with appropriate care provider, recognize when to escalate to higher level of care. Uses appropriate documentation to reflect nursing assessment, plan, interventions and patient response to care. Provides patient education about common medical and mental health concerns and available treatment options. Provides guidance and education to patients about how to navigate the healthcare system, coordinate one's care; Supports patients in developing healthcare navigation autonomy. Monitors patients for changes in clinical symptoms and treatment side effects or complications as clinically indicated. If the patient is not participating in treatment, the care manager works with the patient and other clinical staff to identify and address the barriers to treatment adherence. Identifies and initiates referrals and coordinates available resources among University Health Services, campus and the community. Plays a lead role in the case management of complex patients within Clinical Services, serving as a resource to clinic nurses, clinicians, or other clinical staff. Manage panel of patients assigned to caseload. Provides referrals and linkage to UHS, campus, and community resources to assure a continuum of care for patients. Maintains resource list/database of important contacts and programs. Supports medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment. Administer intramuscular or subcutaneous injections in accordance with UHS policies/procedures. Support students with post-hospitalization care coordination needs. Serve on the Eating Disorders Care Team and Trans Care Team, and help with care coordination needs of these students. Liaise with campus partners, such as Disabled Students Program and Basic Needs Center.


B. Collaboration
  • The Care Manager, as a member of the health care team, is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical providers and, when appropriate, mental health providers. Works directly with all clinic providers, support staff, other professionals and supervisors to support the patient in receiving all applicable integrated health services required to improve the patient's physical and mental health. Supports colleagues by offering and accepting assistance, sharing information and services, and utilizing resources effectively. Prepares difficult or complex cases for review by team members seeking additional expertise (e.g., psychiatry, etc ) as indicated. Participate in regularly scheduled caseload consultation with medical and behavioral health providers. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan.


C. Communication
  • Demonstrates an active investment in organizational and unit goals to improve unit/organization and goals to improve good customer service. Promotes the development of collaborative relationships with peers and other health professionals by articulating practice issues and communicating through appropriate organizational channels. Assumes responsibility for obtaining and sharing relevant information to enhance teamwork and the delivery of quality patient care including collaboration in the preparation of patient education materials. Participates in staff meetings. Demonstrates concern, courtesy and empathy for the customer. Assesses learning needs and utilizes principles of teaching and learning in the education of patients and colleagues. Handles conflict situations appropriately and works towards resolution. Implements University and UHS policies and procedures to assure a consistently high level of patient care, safety and customer service Provides clear communication of plans of intervention for patients. Assists with monitoring and tracking students as they move through University Health Services and referral to off-campus treatment. Routinely conveys information from contacts with the patients to the appropriate care providers through the Electronic Medical Records system.


D. Population Health Duties
  • Track patient follow-up and clinical outcomes using population management tools. Identify trends within populations and ensure access to needed resources and care.


LEADERSHIP/PROFESSIONAL GROWTH
  • Recognizes and functions as a key member of the health care team - provides clinical leadership as it relates to care management while functioning as a clinical resource. Takes a leadership role in referral coordination process and leads clinical referral team. Supports the evaluation and continuous improvement of the collaborative care model at UHS. Makes recommendations to the clinical management team and implements decisions. Exhibits behaviors that support high standards for nursing practice, professional accountability and teamwork, and which contribute positively toward unit goals. Contributes to the improvement of nursing practice. When assigned, participates in the orientation of new employees and acts as preceptor providing comprehensive instruction, training and in depth guidance in a positive manner. Demonstrates and supports professionalism in nursing practice. Takes responsibility for own educational needs. Participates in nursing committees and the development of nursing protocols. Assists in identifying nursing staff learning needs and issues in nursing practice, and may be asked to prepare materials to share with other nurses. Actively contributes to Quality Improvement activities. Facilitates problem solving by the contribution of concerns and/or ideas for improving efficiency and working relations.


ORGANIZATIONAL RESPONSIBILITIES
  • Adheres to principles of quality, customer-oriented standards. Demonstrates professionalism, courtesy, and sensitivity in all interactions with patients and staff. When assigned, actively participates on UHS committees and/or the development of new programs/ideas. Documents and refers administrative inquiries, patient complaints/concerns to appropriate supervisors. Knowledgeable of patient rights and responsibilities, rules of confidentiality and patient privacy. Readily accepts additional responsibility including assisting with the evaluation of unit operation/efficiency. Maintains patient records appropriately including confidentiality and completeness of notes. This includes written and electronic records. Knowledgeable of UHS and campus departments, services, locations. Knowledgeable of operational, administrative, business, and personnel policies. Maintains detailed knowledge of Clinical Services policies and procedures. Works collaboratively with University Health Services staff. Demonstrates flexibility and adaptability as health care practices and related technology change.


OTHER RESPONSIBILITIES
  • Other duties as assigned.


Required Qualifications
  • Minimum of 3 years of nursing practice in an outpatient setting or in nursing case management. Working knowledge of common mental health and/or substance use disorders. Working knowledge of evidence-based psychosocial approaches for common behavioral health concerns, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation). Knowledge of psychopharmacology for common mental health disorders. Knowledge of common health conditions affecting the college-age population. Demonstrated ability to collaborate and communicate effectively in a team setting. Ability to maintain effective and professional relationships with patients and other members of the care team. Demonstrated ability to implement and assist in the development of new or revised clinical and administrative protocols or guidelines. Strong ability to obtain an adequate history with excellent assessment and communication skills and the ability to recognize deviations from normal findings. Demonstrated knowledge of and ability to apply principles of triage in outpatient, ambulatory medicine setting. Proven ability to initially assess laboratory, radiology, and outside test reports and make appropriate and timely consultation with care providers regarding these results. Ability to document process of care appropriately, concisely, and clearly including patient identifying information, relevant history, assessment, and advice given including the disposition. Proven ability to work under pressure calmly, effectively, and efficiently, coping with variable workloads, intermittent delays and emotionally charged situations. Must be computer literate; skills must support medical applications (for example, electronic medical records & retrieval, appointment scheduling programs, tracking and screening programs).


Education
  • Bachelor's degree, Baccalaureate degree in Nursing or Master's degree in Nursing or higher and/or equivalent training or experience.


Licenses or Certifications
  • Possession of a valid California Registered Nurse license. Certification in Basic Life Support BLS/CPR certificate.


Preferred Qualifications
  • Minimum of 5 years of nursing practice in an outpatient setting or in nursing case management.
  • Experience working with individuals around whole-person health including behavioral health concerns.
  • Experience working in an outpatient settings, preferably student health.
  • Experience with case management.


Salary & Benefits

This position is a non-exempt, bi-weekly paid position. Hourly pay for this role is commensurate with experience within the range of $60.50 - $73.78 (Step 1.0 - Step 11.0). For information on the comprehensive benefits package offered by the University visit:

Other Information
  • This is a career, 100% full-time, 40-hours per week appointment, eligible for full UC benefits.

This position is governed by the terms and conditions in the agreement for the Nurse Unit (NX) between the University of California and the California Nurses Association (CNA).
