Departmental Overview

UC Berkeley's University Health Services (UHS) is a comprehensive college health service providing fully accredited, primary medical care, counseling and psychological services, and innovative health promotion programs for students, faculty, and staff. UHS provides on-campus medical care and coordinates supplemental needs for off-campus care through a network of community specialists and hospitals. UHS manages the student health insurance and workers' compensation insurance programs.

Approximately 65,000 visits occur annually including Primary Care, Urgent Care, Occupational Health and Specialty Clinics. Services are designed to minimize the impact of illness, emotional distress and injury on studies and work. Responsibilities

PATIENT CARE

  • Assist and support clinical staff by providing case management and care coordination of patients, especially patients requiring support in multiple areas of mental and physical care.
  • Case management may include: regular phone or in-person patient visits, escalation of health concerns to appropriate clinical staff, identification of patient-centered solutions to overcoming barriers to care, facilitation of internal and/or external referrals, insurance/health system navigation, advocacy and coordination with campus or community partners/resources, coordination of case conferences as needed.
  • Assist patients and providers in development, adherence and follow-through of collaborative care treatment plans, which might include employing behavioral activation, problem solving, goal setting, motivational interviewing and harm reduction techniques.
  • Utilize motivational interviewing to provide support to students in-between their medical or mental health appointments, and encourage adherence to treatment plans.
  • Use EHR data to identify at-risk students seeking care within the medical services, e.g. those with high risk conditions, high utilization, multiple providers, needing help with treatment compliance. Reach out to these patients with a goal of connecting them with a medical team within a medical home model, enlist in case management.
  • Track patient follow-up and clinical outcomes using an EHR registry, ensuring continuity of care and contributing to quality assurance and improvement efforts.
  • Assess patient needs and coordinate appointments/resources to assist patients and the care team with the treatment plan.
  • Work collaboratively with the multidisciplinary UHS care management team.
  • Use appropriate documentation to reflect assessment, plan, interventions and patient response to care.
  • Link patients to educational resources, as needed, regarding common presenting concerns and available treatment options.


COLLABORATION

  • Work directly with the care team, support staff, other professionals and supervisors to ensure that the patient is receiving all applicable integrated health services required to improve the patient's physical and mental health.
  • Support colleagues by offering and accepting assistance, sharing information and services, and utilizing resources effectively.
  • Participate in regularly scheduled caseload consultation with medical and behavioral health providers.


COMMUNICATION

  • Excellent communication skills and experiences in working with patients having varied medical and mental health concerns, especially understanding, engaging, and navigating a broad spectrum of interpersonal communication patterns and clinical resistance - with patients, providers, and third-party consultants, as appropriate.
  • Demonstrates an active investment in organizational and unit goals to improve customer service.
  • Promotes the development of collaborative relationships with peers and other health professionals by articulating practice issues and communicating through appropriate organizational channels.
  • Helps enhance teamwork and the delivery of quality patient care including collaboration in the preparation of patient education materials.
  • Participates in staff meetings.
  • Demonstrates concern, courtesy and empathy for the patient.
  • Handles conflict situations appropriately and works towards resolution.
  • Routinely conveys information from contacts with the patients to the appropriate care providers through the EHR system.


ORGANIZATIONAL RESPONSIBILITIES

  • Adheres to principles of quality, customer oriented standards.
  • Demonstrates professionalism, courtesy, and sensitivity in all interactions with patients and staff.
  • When assigned, actively participates on UHS committees and/or the development of new programs/ideas.
  • Documents and refers administrative inquiries, patient complaints/concerns to appropriate supervisors.
  • Knowledgeable of patient rights and responsibilities, rules of confidentiality and patient privacy.
  • Readily accepts additional responsibility including assisting with the evaluation of unit operation/efficiency.
  • Maintains patient records appropriately including confidentiality and completeness of notes. This includes written and electronic records.
  • Knowledgeable of UHS and campus departments, services, locations.
  • Knowledgeable of operational, administrative, business, and personnel policies.
  • Maintains detailed knowledge of Clinical Services policies and procedures.
  • Works collaboratively with University Health Services staff.
  • Demonstrates flexibility and adaptability as health care practices and related technology change.
  • Implements University and UHS policies and procedures to ensure a consistently high level of patient care, safety and customer service.
  • Develop, maintain, and share internal UHS, campus, and community resources as appropriate with patients and staff to promote efficient quality care.
  • Identify improvements to systems, practices, and procedures and makes recommendations to leadership
  • Other duties as assigned to support the unit, department, entity, and health system organization.


Required Qualifications

Education

  • Masters in Social Work accredited by the Council of Social Work Education


Licenses/Certifications

  • Licensed social worker in the state of CA (or an equivalent/higher licensure level)


Knowledge, Skills and Abilities

  • Minimum of 2 years of social work practice in a clinical setting. Preferably, experience with medical, hospital, or community clinic settings.
  • Experience working with patients who have multiple comorbid medical and mental health concerns and needs.
  • Experience with coordinating patient care with internal and external resources.
  • Demonstrated experience of assisting patients with navigating complex medical/mental health systems, including referral coordination and patient advocacy.
  • Working knowledge of common mental health and/or substance use disorders.
  • Working knowledge of evidence-based psychosocial approaches for common behavioral health concerns, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation). Knowledge of common health conditions affecting the college-age population.
  • Demonstrated ability to collaborate and communicate effectively in a team setting. Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Demonstrated ability to implement and assist in the development of new or revised clinical and administrative protocols or guidelines.
  • Strong ability to obtain an adequate history with excellent assessment and communication skills and the ability to recognize deviations from normal findings.
  • Ability to document the care process appropriately, concisely, and clearly including patient identifying information, relevant history, assessment, interventions, and the disposition.
  • Proven ability to work under pressure calmly, effectively, and efficiently, coping with variable workloads, intermittent delays and emotionally charged situations.
  • Must be computer literate; skills must support medical applications (for example, electronic medical records & retrieval, appointment scheduling programs, tracking and screening programs).
  • Experience in utilizing data to inform decision-making. Ability to perform job duties in sometimes ambiguous situations requiring flexibility and willingness. Interest in continuing development of this new role.


Preferred Qualifications

  • Experience providing care management to university students is preferred.
  • Experience providing care management in a medical setting is preferred.


Salary & Benefits

Under California law, the University of California, Berkeley is required to provide a reasonable estimate of the compensation range for this role and should not offer a salary outside of the range posted in this job announcement. This range takes into account the wide range of factors that are considered in making compensation decisions including but not limited to experience, skills, knowledge, abilities, education, licensure and certifications, analysis of internal equity, and other business and organizational needs. It is not typical for an individual to be offered a salary at or near the top of the range for a position. Salary offers are determined based on final candidate qualifications and experience.

  • The budgeted salary or hourly range that the University reasonably expects to pay for this position is (Step 8.0) $99,985.77 - (Step 12.0) $108,178.08.
  • This is a full-time (40 hours per week), career position that is eligible for full UC benefits.
  • This is an exempt, monthly paid position.
